Insurance

We are happy to assist in coordinating your insurance filing

Our goal is to provide the utmost in assistance and coordination with all of our participating health insurance providers as well as Medicare. We are pleased to be participating providers for Medicare and almost every other major health insurance plan. We also offer a self-pay option for those who do not have or will not be using insurance. 

Preauthorization Requirements

Many health insurance plans require referral authorization from your primary care physician prior to consultation with a specialist. In addition, most plans also require preauthorization before any type of surgical procedure or laser treatment. It is your responsibility to obtain the required referral authorization from your primary care physician.

Failure to obtain a referral authorization, if required, will result in your health plan denying payment for our services. In this case, you will be financially responsible for all services obtained through our office.

If you are uncertain whether your health plan requires referral preauthorization, please call your plan’s customer service representative or your family physician’s office.

Your Insurance Cards

Because we also file all private insurance forms for the convenience of our patients, you must bring your insurance card(s) with you to every visit so that we have accurate information about your coverage. Medicare and Medicaid patients also need to bring their current card(s) at each appointment. If you have questions about our participation in your insurance program or health plan, please call our business office at (713) 799-9975.

Payment for Services

Many plans require a co-payment. We are required to collect co-payments at the time of your visit. We will bill your plan directly for our services. After your plan has paid its part of the charges, we will send you a bill for your portion of the balance. If you do not have medical insurance, you will be expected to pay for your consultation at the time of your visit unless you have made prior arrangements with our business office.

Payment for surgery must be arranged in advance. Our business office staff will work with you to set up a reasonable plan of payment so that you can receive appropriate treatment without delay. Our office accepts credit card payments through Visa and MasterCard. Our billing staff handles your insurance questions and claims submission, including workers’ compensation and all surgical procedure claims. If you have Medicare or an insurance company (HMO/PPO) that we process, deductible and/or co-payment will be expected before you leave our office. HMO patients are required to have the proper referral forms prior to being seen.

Holiday Hours

Retina & Vitreous of Texas will be closed on Tuesday, December 24th, and Wednesday, December 25th, in observance of Christmas. A provider will remain on call for emergencies. We wish you a safe and joyful holiday! Regular business hours will resume on Thursday, December 26th.
